When applying for Social Security Disability (SSD) benefits, you will receive communications from the Social Security Administration stating whether your benefits are approved or denied.
When your SSD benefits claim is denied, if you don't agree with the decision, the Social Security Administration offers an appeal process. The process for appealing a Social Security decision includes submitting a Social Security Request for Reconsideration through Form SSA-561.
What Should You Include in a Social Security Request for Reconsideration?
When applying for a Social Security Request for Reconsideration, you will need to include certain information. Information that may be required for your Social Security Administration Request for Reconsideration includes, but may not be limited to:
- Name,
- Social Security Number,
- Address,
- Phone Number,
- Date of Denial Decision (this is typically printed above your claim number on the decision letter from the Social Security Administration),
- Attorney's name, address and phone number (if applicable),
- Name, address and phone number of a friend or relative who is aware of your medical condition
- Changes related to your medical condition and new medical conditions
- Date of service and contact information for all medical providers, including treatments and tests since previously provided medical evidence
- Name(s) of current medications (either prescribed or over-the-counter), reasons for taking medication(s), any experienced side effects and the name of the medical provider who prescribed or recommended the medication(s)
- Describing any changes in your day-to-day activities, education or work

If you applied for Social Security Disability and your benefits claim was denied for medical reasons, you can request an appeal through ssa.gov.
As part of your Social Security Request for Reconsideration for medical reasons, you may need to provide supporting documents. The Social Security Administration lists examples of supporting documents, including:
